What the form collects
- Whether you are a U.S. citizen
- Whether you have used tobacco in the last 12 months
- Date of birth
- Name
- Street address, an optional apartment, unit, or lot, city, state, and ZIP code
- Phone number, if you submit one
- Ad parameters already in the link you clicked, such as utm tags, fbclid, or gclid
Partials and completed requests
Once your name, date of birth, and address are filled in, those details are saved even if you leave before entering a phone number. If you later submit a valid phone number, that is saved as a completed request. The information is emailed to the business operating this site so a licensed insurance agent can contact you about final expense coverage. There is no obligation to buy.
Meta pixel
If a Meta pixel is configured, a Lead event is sent only after a completed form submit succeeds. The pixel does not load when the page opens, when a partial quote is saved, or when you tap Call now. The pixel is Meta’s (Facebook and Instagram). Meta’s own policy describes how they handle that event.
